Job Description
Remote RN case manager coordinating hospital-to-home transitions for Alignment Health, a senior-care organization. Creating discharge plans, connecting patients with providers and community resources, and monitoring outcomes.
Responsibilities:
- Manage and coordinate health care for members with complex and chronic care needs within the scope of licensure
- Assess, develop, implement, coordinate, monitor, and evaluate care plans across the care continuum
- Coordinate and monitor member progress and services to ensure consistent, cost-effective care compliant with policies and regulations
- Create individualized discharge plans with patients, families, and the care team
- Coordinate care with providers, rehabilitation facilities, and home health agencies
- Monitor patient progress and adjust care plans as needed
- Educate and empower patients and families for self-care and follow-up
- Advocate for patient needs and connect them to community support services
- Participate in care conferences, quality improvement initiatives, and interdisciplinary collaboration
- Perform duties mostly telephonically
Requirements:
- 5 years of clinical case management experience, or an equivalent combination of education and experience
- Active, unrestricted RN license in California
- Willingness to obtain other state licenses
- Knowledge of Medicare Managed Care Plans, insurance regulations, and community resources
- Strong communication, problem-solving, and organizational skills
- Ability to perform the essential physical functions, including standing, walking, sitting, using hands, reaching, and frequently lifting or moving up to 10 pounds
- Bilingual Spanish requirement indicated by the job title
