Job Description
Telehealth Physician delivering preventive and chronic care to Medicare seniors for Belle. Conducting routine virtual visits, managing care plans, and coordinating with integrated clinical teams.
Responsibilities:
- Provide routine telehealth visits with Medicare patients, including E/M and Chronic Care Management
- Create and manage personalized care plans for ongoing conditions
- Collaborate with the integrated care team, including nurse case managers, NPs, and CHWs
- Ensure thorough documentation in the EHR and compliance with quality measures
- Engage patients in preventive care to reduce avoidable complications
Requirements:
- MD or DO
- Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active medical license in Georgia
- Multi-state licensure a plus
- Credentialed with major plans: UHC, Aetna, Humana
- Strong interest in preventive care, senior health, or chronic condition management
- Tech-savvy and comfortable practicing via telehealth
- Collaborative, mission-driven mindset with excellent communication skills
