Job Description
Care Navigator guiding Canadian older adults and caregivers through Sailor Health’s mental-health intake process. Matching patients with therapists and providing compassionate care coordination remotely.
Responsibilities:
- Serve as the first voice patients or their families hear when reaching out to Sailor Health
- Guide older adults and caregivers through the intake process with warmth, clarity, and professionalism
- Provide patient support, care coordination, and emotional guidance
- Help each new patient navigate entry into care
- Match patients with the right therapist
- Support patients and caregivers throughout the intake and care-navigation journey
- Help older adults and their families feel seen, supported, and cared for
- Take initiative and help shape key processes within a small, agile team
Requirements:
- Full-time availability
- Empathetic and clear communication
- Active listening and ability to make people feel at ease during difficult or emotional conversations
- Highly organized and detail-oriented
- Ability to manage multiple patients at different stages without letting tasks slip
- Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot
- Ability to quickly learn new software
- Care deeply about improving care for older adults
- Experience in healthcare, customer service, or administrative support is welcome but not required
- Must be based in Canada
- Comfortable working Eastern Time Zone hours
- Willingness to work one of the listed Monday–Friday, Wednesday–Sunday, or Saturday–Wednesday shifts, from 9 a.m.–6 p.m. ET or 12 p.m.–9 p.m. ET
