Job Description

Care Navigator supporting Curana Health’s senior-care providers and patients remotely. Managing EHRs, scheduling, patient communications, records, and provider coordination.

Responsibilities:

  • Address patient and durable power of attorney (DPOA) inquiries via telephone
  • Respond to patient or caregiver messages received via the Curana Patient Portal
  • Assist patients with scheduling follow-up appointments with Curana Providers or specialists
  • Manage electronic health records (EHR) and ensure accurate and up-to-date patient records
  • Coordinate documents needed for provider review or signature
  • Facilitate provider orders and escalate findings
  • Maintain patient rosters for patients enrolled in APCM and GUIDE
  • Support provider scheduling
  • Assist with prior authorizations
  • Obtain patient records and diagnostic test results
  • Answer and manage incoming calls professionally and courteously
  • Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
  • Perform other duties as assigned

Requirements:

  • High school diploma or equivalent
  • 1+ years of experience working in a medical office, Senior Living Community engagement, or other related fields
  • 1+ years of experience in Electronic Health Record (EHR) documentation or other practice management tools
  • Extensive understanding of medical terminology
  • Ability to interpret medical records, lab results, and appointment notes
  • Basic knowledge of reviewing patient screening tools and identifying changes over time
  • Ability to work in an environment free of distractions
  • Excellent organizational and time management skills with the ability to prioritize tasks
  • Skilled at handling multiple tasks simultaneously
  • Proficient computer skills and ability to adapt to various technology platforms
  • Prior experience with virtual triage preferred
  • Bilingual or multilingual communication skills preferred
  • Reliable high-speed internet connection
