Job Description
Care Navigator guiding older adults and caregivers through Sailor Health’s mental health intake process. Coordinating therapist matching and providing compassionate patient support remotely across Canada.
Responsibilities:
- Serve as the first voice patients or their families hear when contacting Sailor Health
- Guide older adults and caregivers through the intake process
- Provide patient support, care coordination, and emotional guidance
- Help patients match with the right therapist
- Ensure patients feel supported, confident, and understood throughout their entry into care
- Manage patients at different stages of the process
- Take initiative and help shape key processes within a small, agile team
Requirements:
- Canada-based only
- Empathetic and clear communicator
- Active listening and ability to handle difficult or emotional conversations
- Highly organized and detail-oriented
- Ability to manage multiple patients at different stages
- Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot
- Ability to learn new software quickly
- Interest in mental health and healthcare access
- Healthcare, customer service, or administrative support experience welcome but not required
- Willingness to work Eastern Time Zone hours
- Availability for one of the specified 9a–6p ET or 12p–9p ET shift schedules
