Job Description

Ambulatory Clinical Pharmacy Specialist redefining patient care at Shields Health Solutions. Collaborating in clinic settings to optimize medication management and enhance patient outcomes.

Responsibilities:

  • Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
  • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
  • Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
  • Support quality-improvement and REMS initiatives.
  • Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.

Requirements:

  • Hold a Doctor of Pharmacy (PharmD)
  • Must be a licensed pharmacist in Oregon
  • Certified as an HIV Specialist (AAHVIP)
  • Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency and at least 1 year of ambulatory practice under a CPA or at least 3 years of ambulatory practice with disease state management
  • Strong communication, organization, and tech skills
