Job Description
Remote clinical pharmacist delivering medication management for Shields Health Solutions’ integrated specialty-pharmacy patients. Supporting virtual assessments, therapy optimization, and collaborative care across the Centralia/Olympia/Lacey area.
Responsibilities:
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement (CPA) and promote seamless medication coordination
- Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate
- Support quality-improvement and REMS initiatives
- Support teammates with complex prior authorizations, improve workflows, educate patients, and maintain ongoing clinical development
- Build relationships with patients and providers through virtual patient assessments and evidence-based treatment-plan monitoring
Requirements:
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Completion of a PGY2 Ambulatory Care Residency, or completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA), or at least 3 years of ambulatory practice with disease state management
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
- Solid computer skills required, including proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint)
- Ability to navigate the internet and web-based platforms
- Data entry and computer skills will be evaluated through a pre-employment assessment
