Job Description
Remote Clinical Triage Nurse assessing patient symptoms and test results for HarmonyCares’ in-home primary care services. Coordinating urgent care guidance, provider communication, and patient follow-up.
Responsibilities:
- Triage inbound sick calls from Medical Group patients
- Assess patient symptoms and provide appropriate medical advice
- Determine the level of care required and coordinate care with the care team
- Review clinical test results and summarize findings for providers
- Prioritize and respond to inbound phone calls, voicemails, and test results
- Conduct thorough phone assessments using probing questions and the EMR
- Apply evidence-based guidelines to assign triage priority
- Identify crises and connect patients to emergency services according to protocol
- Educate patients and caregivers on health maintenance, disease prevention, self-care, medications, and follow-up
- Perform outbound calls communicating provider orders
- Collaborate with care teams to complete follow-ups and promote quality patient outcomes
- Analyze results data and communicate needs, patterns, and variances to providers
- Enter patient results and quality metrics
- Address and escalate patient complaints, grievances, and concerns
- Participate in projects and quality improvement initiatives
- Perform additional duties assigned by the Clinical Triage Manager and/or Director
Requirements:
- Graduate of an approved or accredited nursing education program as a Registered Nurse
- Current valid RN license
- Must be able to obtain and maintain RN license within all service states within 6 months from hire
- Satisfactory completion of required nursing continuing education requirements
- Ability to react decisively and quickly in urgent and emergent situations
- Strong comprehension of medical terminology and pharmaceuticals
- Demonstrated experience with multitasking
- Strong written and spoken communication skills
- Demonstrated proficiency using an electronic health records (EHR) system
- Ability to communicate effectively with providers, patient care teams, patients, patients’ family members, and facility staff
- Ability to work remotely and independently
- Knowledge of basic principles, practices, and techniques in primary healthcare
- Strong organizational skills and attention to detail
- Conflict resolution skills
- Preferred: familiarity with Medicare and Medicaid insurances and programs
- Preferred: 2+ years of experience as an RN
- Preferred: contact center experience
- Preferred: nurse triage experience
- Preferred: experience in Aprima (EHR)
- Preferred: multistate license (MSL)
