Job Description
Community Health Worker connecting Elevance Health patients with care teams and community resources. Conducting outreach, addressing social determinants, and documenting care support.
Responsibilities:
- Serve as a liaison/connector between patients, care teams, and the community
- Help bridge conversations with patients and remove barriers to health and social services
- Conduct telephonic outreach to patients for appointment scheduling, needs assessment, and care gap closure
- Identify social determinants of health affecting patients’ health and well-being
- Collaborate with social workers and/or care managers to develop action plans
- Build trusting relationships through patient support and advocacy
- Assist patients with health-related services and community resources
- Accompany patients to specialist appointments and assist with enrollment forms
- Facilitate communication among patients, families, colleagues, and community-based organizations
- Document patient interactions in medical records
- Attend required meetings
- Travel to worksites and other locations as necessary
Requirements:
- H.S. diploma or equivalent
- Minimum of 1 year of experience in a healthcare, community-based, or social work environment
- Experience utilizing electronic medical records
- Any combination of education and experience providing an equivalent background
- Ability to work an 8-hour shift between 8am and 6pm Pacific or Mountain Time
- Ability to work within commuting distance from an office unless accommodation is granted
- Bilingual preferred
- Certified Community Health Worker preferred
- Ability to travel to the worksite and other locations as necessary
- COVID-19 and Influenza vaccination may be required for certain patient/member-facing roles
