Job Description

Discharge Planning Coordinator guiding members to step down from hospital care. Coordinating services and discharge planning while advocating for community resource access.

Responsibilities:

  • Serve as a point of contact on Hospital/ED calls to assist with discharge planning and coordination of care needs following an ED/inpatient admission
  • Provide short-term care coordination on a case-by-case basis, which may include working with management to remove barriers to expedite ED discharge; for members who are inpatient, safe and timely discharge will be facilitated by providing viable and appropriate recommendations
  • Link and/or refer members, guardians, hospital/ED staff and stakeholders to various services, providers or community resources specific to the population being served
  • Participate on various cross-functional projects or tasks
  • Review and maintain data, information, and documentation
  • Work collaboratively with internal departments to identify, communicate and track needed services, providers, and resources

Requirements:

  • High School Diploma/GED and six (6) years’ experience working with the Adult and Child behavioral health/mental health, developmental disabilities or substance use population
  • OR Associate’s degree and four (4) years’ experience working with the Adult and Child behavioral health/mental health, developmental disabilities or substance use population
  • OR Bachelor’s degree and minimum two (2) years’ experience working with the Adult and Child behavioral health/mental health, developmental disabilities or substance use population
  • OR Equivalent combination of education/experience
  • Must have a valid driver’s license
  • Must reside within North Carolina
  • Must be able to travel within catchment as required
  • Preferred: Experience providing crisis intervention or crisis response to individuals within the behavioral health/mental health, developmental disabilities or substance abuse population
  • Fluent Spanish speaker
