Job Description
In-home Nurse Practitioner or Physician Assistant conducting health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, performing physical evaluations, and delivering preventive screenings.
Responsibilities:
- Provide approximately 45-minute in-home health evaluations
- Engage with health plan members in their homes and build trusted relationships
- Review medications and medical history
- Perform physical evaluations
- Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening
- Use an iPad and clinical workflow for each evaluation
- Visit people nationwide to connect them with appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services products
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Nurse Practitioner or Physician Assistant qualification
- Able to cover the specified primary locations
- Able to travel locally and within licensed states, as needed
- Able to conduct virtual visits as needed
- Able to safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
