Job Description
Part-time NP or PA conducting 45-minute in-home health evaluations for CVS Health’s Signify Health. Reviewing medical histories, performing physical assessments, and delivering diagnostic services across Livingston and Rochester.
Responsibilities:
- Provide approximately 45-minute In-Home Health Evaluations
- Engage one-on-one with health plan members in their homes
- Review medications and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
- Use an iPad and intuitive clinical workflow for each evaluation
- Visit people nationwide as part of efforts to connect them with appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services products
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Livingston, Rochester and surrounding area
- Ability to travel locally and within licensed states as needed
- Ability to conduct virtual visits as needed
- Ability to safely perform duties in homes containing household pets, secondhand smoke and other potential airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
