Job Description
Physician conducting in-home health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, performing physical exams and delivering preventive diagnostics across Vermont and licensed states.
Responsibilities:
- Provide in-home health evaluations in members’ residences
- Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals and/or flights
- Conduct approximately 45-minute visits with one-on-one health discussions
- Review medications and medical history
- Perform physical evaluations
- Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services
- Build trusted relationships and help improve health outcomes
- Make referrals to members’ health plans and encourage involvement of loved ones when appropriate
- Ensure primary care physicians are aware of members’ health and quality of life
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Able to obtain and maintain multi-state licenses in coverage area, as required
- MD or DO qualification
- Able to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, airborne allergens or irritants
- Willingness and ability to travel locally and within licensed states, up to 100% of the time
- Ability to conduct virtual visits as needed
