Job Description
Medical Director overseeing AI-driven chronic condition management and hospital-at-home care. Guiding clinical quality, protocols, virtual care operations, and technology-enabled patient outcomes.
Responsibilities:
- Provide physician leadership for hypertension, diabetes, and CHF programs
- Maintain oversight of evidence-based clinical protocols and care pathways
- Review and recommend updates to clinical guidelines based on new research and evolving standards of care
- Support escalation of complex clinical cases
- Partner with Nursing leadership to optimize care delivery workflows
- Collaborate with Product and Engineering teams to develop AI-enabled clinical workflows, clinical automation, agentic prescription/titration, and decision support
- Review new product features for clinical safety, usability, and effectiveness
- Help define future clinical capabilities improving patient engagement and outcomes
- Monitor program performance and clinical quality metrics
- Identify opportunities to improve patient outcomes, engagement, and operational efficiency
- Support quality improvement initiatives and clinical performance reviews
- Help develop reporting for payor clients, including clinical outcomes and utilization trends
- Ensure clinical programs align with evidence-based medicine and regulatory requirements
- Participate in clinical governance committees and quality reviews
- Support accreditation, compliance, and audit activities
- Collaborate with Legal and Compliance on clinical policies and documentation
- Provide education to internal teams regarding clinical best practices and emerging evidence
Requirements:
- MD with an active, unrestricted license to practice medicine in the United States
- Board Certification in Internal Medicine, Cardiology, Endocrinology, or a related specialty
- Minimum of 5 years of post-residency clinical practice
- Demonstrated clinical expertise in the management of hypertension, diabetes, congestive heart failure (CHF), and other chronic conditions common to the Medicare population
- Experience developing, implementing, or overseeing evidence-based clinical protocols and care pathways
- Experience working with interdisciplinary care teams, including physicians, nurse practitioners, nurses, and care coordinators
- Experience with digital health, virtual care, Remote Patient Monitoring (RPM), or Chronic Care Management (CCM), preferred
- Knowledge of value-based care models, CMS quality programs, HEDIS, and Star Ratings
- Experience working with health plans or provider organizations
- Familiarity with AI-enabled clinical decision support tools and healthcare technology
