Job Description
In-home Nurse Practitioner or Physician Assistant conducting medication reviews, physical evaluations, and preventive screenings. Serving Signify Health members across Manhattan and surrounding areas.
Responsibilities:
- Provide approximately 45-minute In-Home Health Evaluations
- Engage with health plan members in their homes and build trusted relationships
- Review medications and medical history
- Perform physical evaluations
- Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Visit people nationwide to connect them with appropriate care
- Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
- Conduct virtual visits as needed
- Travel locally and within licensed states as needed
- Work as part of a national network of purpose-driven clinicians
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Manhattan and surrounding area
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
- Ability to travel locally and within licensed states as needed
- Ability to conduct virtual visits as needed
