Job Description
Remote Registered Dietitian serving a Nashua, New Hampshire long-term care community for Nutrition that Works, LLC. Completing assessments, MDS documentation, tube-feeding calculations, and wound-related recommendations.
Responsibilities:
- Serve the long-term care community located in Nashua, NH
- Complete clinical nutrition assessments
- Document using the Nutrition Care Process in the EMR system
- Collaborate with the interdisciplinary care plan team
- Complete MDS - Section K
- Calculate TF
- Make recommendations for wounds and significant weight changes
Requirements:
- Must have NH LDN
- LTC experience preferred
- Credentialed by the CDR
- Dietetic licensure
- Positive attitude
- Client centered
- Good communication skills
