Job Description

Remote clinical pharmacist supporting Shields Health Solutions’ medication therapy management program. Providing phone-based patient assessments, counseling, and medication safety oversight for New York patients.

Responsibilities:

  • Support the site Therapy Management program by providing pharmaceutical care services to patients
  • Perform phone-based assessments on disease-state management and drug-therapy education based on medication risk level and approved clinic protocol
  • Identify, resolve, and prevent medication-related problems and overcome barriers to adherence
  • Review and evaluate medication orders for accuracy, appropriateness, and safety
  • Provide medication counseling to patients
  • Collaborate with healthcare providers to develop and monitor medication therapy plans
  • Conduct patient-care assessments and identify opportunities to optimize drug therapy
  • Participate in interdisciplinary collaboration and provide clinical recommendations
  • Monitor and manage drug interactions, adverse effects, and therapeutic outcomes
  • Maintain accurate patient records and document interventions
  • Stay current with pharmacotherapy developments and contribute to clinical protocol development
  • Ensure compliance with state and federal regulations and accreditation standards
  • Assist in training and mentoring new hires, pharmacy students, and pharmacy liaisons (technicians)

Requirements:

  • Applicant must hold a Doctor of Pharmacy (PharmD) degree
  • Applicant must be a licensed pharmacist in New York
  • Strong clinical knowledge relevant to the site’s Therapy Management program, primarily diabetes mellitus and/or weight management
  • Strong clinical knowledge and experience in a healthcare or hospital setting
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a healthcare team
  • Attention to detail and commitment to patient safety
  • Commitment to continuing education and professional development
  • Solid computer skills required
  • Proficiency in Microsoft Office tools, including Outlook, Teams, Word, Excel, and PowerPoint
  • Ability to navigate the internet and web-based platforms
  • Data entry and computer skills will be evaluated through a pre-employment assessment
