Job Description

Bilingual remote nurse delivering app-based chronic-condition care at Salvo Health. Supporting behavior change, patient engagement, and personalized health education for gut and metabolic conditions.

Responsibilities:

  • Provide exceptional care, disease management, and health education to patients
  • Support individual patient goal setting asynchronously to help manage chronic conditions
  • Create personalized action plans to reduce or eliminate unwanted behaviors
  • Help clients understand their motivations and create behavior change plans
  • Conduct regular patient outreach based on needs and preferences to support engagement and behavior change
  • Coordinate with clinical team members to provide an exceptional patient experience
  • Develop and maintain professional, support-oriented relationships with patients and team members
  • Create and distribute health education materials to individual members as necessary
  • Collaborate with a cross-functional product team to develop and improve the in-app patient experience

Requirements:

  • Compact state license required; additional licensing may be needed
  • Spanish speaking, reading, and writing required
  • 2+ years of experience as a Licensed Practical Nurse or with any nursing license
  • 2+ years of experience in patient-facing or customer-facing roles
  • Excellent customer relations, written communication, and verbal communication skills
  • Knowledge of medical terminology
  • Proficiency in general medical office procedures
  • Familiarity with Slack, Coda, Google Workspace, and similar digital applications
  • Strong analytical and proactive problem-solving skills
  • Self-motivated, results-oriented, and strategic thinker
  • Personal passion for health and wellness topics
  • Must be authorized to work in the United States
  • Experience in telehealth or a healthcare startup preferred
  • Experience in GI, weight management, and/or anti-obesity medications such as GLP-1s is a bonus
