Job Description
Remote Care Nurse II providing virtual assessments, care planning, and chronic-condition support. Connect America uses connected-care technology to help patients live safely and independently at home.
Responsibilities:
- Conduct thorough remote patient assessments via phone or video conferencing
- Develop, implement, and update individualized care plans with patients and healthcare teams
- Monitor patient progress and treatment-plan adherence
- Provide education and guidance to promote self-care and disease management
- Coordinate with physicians, specialists, and other healthcare professionals for seamless care delivery and timely interventions
- Document patient interactions, clinical findings, and care activities accurately in electronic health records
- Respond promptly to patient inquiries and urgent health concerns, escalating issues as necessary
- Participate in quality improvement initiatives and continuing education
- Use telehealth technologies to monitor patient progress and support chronic-condition self-management
- Help reduce hospital readmissions and improve quality of life
Requirements:
- Current and unrestricted Registered Nurse (RN) license in the United States
- Minimum of two years of clinical nursing experience
- Proficiency with electronic health record (EHR) systems and telehealth technology platforms
- Strong communication skills for engaging patients and interdisciplinary teams remotely
- Ability to work independently in a remote environment while managing multiple patient cases efficiently
- Certification in Case Management (CCM) or Telehealth Nursing (CTN) is highly desirable
- Experience working in a remote or virtual care setting
- Background managing complex chronic conditions such as diabetes, heart failure, or COPD
- Familiarity with healthcare regulatory requirements and quality standards related to telehealth services
- Additional training or education in patient education, motivational interviewing, or behavioral health
