Job Description
Case Manager delivering virtual intensive outpatient recovery services for Montana adults with substance use and serious mental health conditions. Coordinating treatment plans, Medicaid support, and telehealth care.
Responsibilities:
- Assess and evaluate adults to determine appropriateness and need for care management services
- Assist consumers with entry into services and monitor progress throughout the continuum of care
- Develop, coordinate, and monitor Individual Treatment Plans (ITPs)
- Complete treatment plans within 5 encounters or 21 days, whichever is sooner, and update clinical assessments annually
- Coordinate treatment team meetings and ensure treatment plans are outcome-based and recovery-focused
- Educate consumers on recovery markers and incorporate them into treatment plan evaluation
- Screen consumers for Medicaid eligibility and assist with enrollment as applicable
- Complete required documentation, reports, and applications within established timelines
- Complete SDMI designation and Level of Impairment (LOI) before initiating case management for consumers with mental health diagnoses
- Collaborate with internal team members and external community providers
- Meet bi-weekly with the Program Supervisor to review consumer cases and service delivery
- Attend required trainings, workshops, and conferences and share relevant learning with team members
- Deliver services primarily via the agency's virtual IOP telehealth platform
- Maintain telehealth documentation reflecting clinical appropriateness for remote delivery
- Provide Community Based Psychiatric Rehabilitation Services (CBPRS) when applicable
- Deliver CBPRS skills training, including description, modeling, practice, and performance feedback
- Consult with family members or key support persons as clinically indicated
- Provide individual or group CBPRS within specified duration and group-size limits
- Document rationale for telehealth CBPRS and attempts to identify local supports when applicable
- Perform other duties as assigned
Requirements:
- Bachelor's degree in Social Work, Psychology, or a related human services field from an accredited college; or an equivalent combination of education and at least two years of full-time experience serving adults with mental illness, substance use disorders, or a related human services population
- Lived experience as a consumer, provider, or advocate of mental health services may be considered, with equivalency documented per agency policy
- Reliable internet connection and a private, confidential workspace suitable for telehealth service delivery
- Working knowledge of Montana Medicaid behavioral health regulations, SDMI criteria, Level of Impairment (LOI) documentation standards, and targeted case management requirements
- Ability to assess and respond to the clinical and functional needs of adults with SUD and/or serious mental illness
- Familiarity with community resources and ability to connect consumers with medical, social, vocational, recreational, or other support services
- Effective verbal and written communication skills sufficient to build trusting therapeutic relationships and produce compliant clinical documentation
- Ability to work collaboratively within a multidisciplinary treatment team and with external community providers
- Flexible scheduling, including occasional evening hours, to meet consumer needs
- Attendance at required staff meetings, trainings, workshops, and conferences
