Job Description
Enrollment Specialist enrolling patients in HealthSnap’s chronic care management and remote monitoring programs. Guiding referrals through consent, device shipment, and enrollment.
Responsibilities:
- Enroll patients in chronic care management and/or remote patient monitoring programs
- Partner with cardiology, internal, primary, and family medicine physician groups and healthcare systems
- Reach out to identified patients and enroll them in Care Management Programs
- Explain program benefits, patient financial responsibility, consent requirements, device shipment, and program enrollment
- Ensure a seamless, complete, and effective enrollment process
- Serve as the initial point of contact for pending enrollee calls
- Manage a high open caseload until enrollment is complete
- Provide exceptional customer service and a positive experience for prospective patients and caregivers
- Navigate multiple systems and portals to input patient enrollment information
- Achieve monthly new enrollee goals
- Perform additional duties as assigned
Requirements:
- Experience in a high-volume inbound and outbound call environment required
- Enrollment/Medical Sales experience preferred
- Experience in patient enrollment and call center experience
- Poise and patience when dealing with patients
- Comfortable communicating with patients over the phone
- Ability to overcome objections
- Good problem-solving skills
- Keen attention to detail
