Job Description

Outbound enrollment specialist enrolling Puerto Rican patients in Scene Health’s medication-adherence and lifestyle-support programs. Guiding patients through outreach, onboarding, app setup, and care-resource connections.

Responsibilities:

  • Conduct high-volume outbound outreach to eligible program participants and support patient enrollment in Scene’s high-touch health program
  • Communicate with patient prospects via phone calls, SMS/text, and email to introduce Scene services
  • Support inbound and outbound communications, primarily outbound calling and texting
  • Identify patient needs and match them with programs and services aligned with insurance benefits and health needs
  • Use empathetic listening and handle frustrations and escalated patient calls effectively
  • Guide patients through enrollment using motivational support and technical knowledge
  • Maintain accurate patient demographic information in Salesforce
  • Address patient objections, questions, and technical barriers during onboarding
  • Assist with app downloads, technical setup, troubleshooting, and Spotlight app usage
  • Connect patients with primary care providers, lab services, pharmacies, and health plan benefits
  • Maintain detailed records of patient interactions, enrollment status, and follow-ups
  • Establish performance goals with management and use time management and critical thinking to achieve them
  • Provide feedback on patient challenges and recommend process improvements
  • Build strong relationships with patients and the Care Team, including nurses, pharmacists, and managers
  • Perform other duties as assigned

Requirements:

  • Must be a legal resident of Puerto Rico and maintain Puerto Rico residency throughout employment
  • Must be legally authorized to work in Puerto Rico/United States without employer sponsorship; visa sponsorship is not available
  • Must be fluent in both English and Spanish, with the ability to read, write, speak, and comprehend both languages proficiently
  • 3+ years of experience in health education, patient engagement, or a related healthcare role
  • Background in health coaching, public health, or community health work
  • Strong interpersonal skills with the ability to engage in warm, empathetic conversations
  • Excellent verbal and written communication skills
  • Experience conducting outreach via phone, SMS, and video communication
  • Ability to explain complex health concepts in a simple, motivating way
  • Proficiency with database systems and mobile applications
  • Weekend availability
