Job Description
SSM Health intake nurse reviewing hospice, home health, and infusion referrals. Coordinating admissions, eligibility, patient transitions, and multidisciplinary care planning.
Responsibilities:
- Communicate with patients and families about continuum of care services
- Meet and communicate with referral sources, including case management, social work, clinics, providers, and SNF/ALF facilities
- Coordinate care between continuum of care departments, branch departments, and business development
- Review eligibility and charts to determine appropriateness for continuum of care services
- Review and analyze patient referrals for home health, hospice, and infusion services
- Coordinate services to support smooth transitions from hospital to home care
- Communicate patient needs and physicians' orders to clinicians to establish plans of care
- Provide educational information to case managers, physicians, hospital staff, and nursing home staff
- Assess patient care needs using hospital records and referral information
- Identify patient and significant-other educational needs
- Ensure compliance with skilled service and homebound regulations of payers
- Pass pending referral information to supervisors and rehabilitation services in a timely manner
- Coordinate initiation of care and obtain appropriate orders for multidisciplinary care
- Reassess patients and records to evaluate care progress and identify trends requiring intervention
- Communicate with supervisors about managed cases awaiting initiation of care
- Assess hazardous or unsafe environmental factors and respond according to policy
- Perform other duties as assigned
Requirements:
- Experienced nurse with Hospice, Home Health, Liaison, or Case Management experience
- Dynamic personality, resilience, energy, and passion for Hospice and Home Health care
- Required to obtain licenses for Missouri, Illinois, Oklahoma, and Wisconsin within 6 months of starting
- Graduate of an accredited school of nursing or education equivalency for licensing
- One year of licensed practical nurse experience
- Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association
- Licensed Practical Nurse (LPN) license issued by the Illinois Department of Financial and Professional Regulation (IDFPR) for Illinois work location
- Licensed Practical Nurse (LPN) license issued by a compact state for Missouri, Oklahoma, and Wisconsin work locations
- Ability to work three weekdays supporting Home Health and every weekend supporting Hospice
- Ability to perform required physical activities, including standing, walking, lifting, carrying, pushing, pulling, patient movement, keyboard use/data entry, occasional driving, and maintaining alertness and safety
