Job Description
Medical Social Worker leading social services for Caring Hand Home Health Solutions’ in-home care. Supervising staff, coordinating patient support, and ensuring compliant clinical documentation.
Responsibilities:
- Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers
- Recruit, onboard, and mentor social workers, providing clinical supervision, observing practice, and supporting professional growth
- Oversee assessment, planning, and evaluation of social services care, ensuring clinical documentation meets state, federal, and ACHC standards
- Partner with the Executive Director on the annual department budget and resource planning
- Build knowledge of community resources and connect patients and referral sources to appropriate support
- Plan in-service training and continuing education
- Contribute to quality improvement initiatives and identify new ways to better serve patients
- Perform other duties as required for patient services
- Provide professional and paraprofessional home health services to clients in their homes, supporting self-care and comprehensive care needs
Requirements:
- A Master of Social Work (MSW) from an accredited program
- An active California license as an LCSW
- At least 3 years of management experience, preferably in home care
- Strong knowledge of healthcare regulations, ACHC standards, and business operations
- Excellent communication skills and the ability to build trust with patients, families, staff, and the community
- A self-directed, organized, and flexible approach—you take initiative and work well with others
