Job Description

Remote Nurse Navigator coordinating follow-up for patients with incidental radiological findings. Supporting Trinity Health care teams through education, referrals, and care navigation.

Responsibilities:

  • Facilitate communication and coordinate care among patients, primary and specialty care physicians, and hospital departments on standardized care pathways
  • Ensure availability, access, and timeliness of services to optimize quality of care, coordination, and patient experience
  • Provide evaluation, education, coordination, and referral to patients identified with incidental radiological findings requiring follow-up
  • Contact patients identified with incidental or unexpected findings and offer recommended follow-up
  • Provide personalized nurse navigation interventions, including assessment of educational level, barriers to learning, and psychosocial factors
  • Ensure appropriate follow-up intervals and avoid duplicate follow-up requests
  • Direct patients to the correct provider based on follow-up recommendations
  • Assist PCPs with appropriate follow-up at radiologist-recommended intervals
  • Obtain additional imaging at the specific request of the PCP
  • Provide education and navigation through the follow-up process
  • Provide referrals and education based on appropriate care
  • Use software programs to track daily and periodic assignments
  • Communicate with local and regional colleagues, Radiology, and Oncology departments to develop the program and maximize patient care
  • Identify problems, offer solutions, and participate in resolution
  • Perform other duties as assigned
  • Maintain knowledge of applicable laws, regulations, Trinity Health compliance requirements, Code of Conduct, policies, and procedures

Requirements:

  • Graduate of an accredited school of Nursing
  • Bachelor of Science Degree in Nursing
  • Three (3) to five (5) years of progressive clinical nursing experience is required
  • Current license to practice as a Registered Nurse in the State of Michigan
  • Additional state-specific licensing may also be required
  • Problem solving skills and ability to initiate creative solutions to problems
  • Demonstrated ability to expertly navigate a variety of software applications
  • Ability to effectively communicate with Medical Staff, departmental staff, Trinity Health department representatives, physicians, other health providers and facilities
  • Working knowledge of Radiology procedures protocols and processes, preferred
  • Knowledge of Windows based programs
  • Effective organizational skills
  • Ability to adapt to frequently changing work priorities and prioritize and balance requirements of working with the System Office and Regional Health Ministries
  • Good manual dexterity to operate a computer keyboard
  • Good hearing for extensive telephone conference calls and in-person communication
