Job Description

Nurse Practitioner delivering fully remote serious-illness care management for Chapters Health System, a nonprofit hospice and palliative-care organization. Providing virtual assessments, prescriptions, care coordination, and family education.

Responsibilities:

  • Provide fully remote virtual serious illness care management to patients and families across home-based, facility-based, and inpatient hospice, palliative care, PACE, and other service lines
  • Consult with medical staff and interdisciplinary team members to ensure optimal patient care
  • Perform virtual assessments of patient, family, and caregiver needs
  • Identify, manage, order, and prescribe medications and equipment as appropriate
  • Adjust and support individualized plans of care within organizational policies and nurse practice acts
  • Assess caregiver ability and confidence, and provide education as needed
  • Answer clinical questions, enter orders and prescriptions, and support patient and family clinical needs
  • Maintain access to EMRs, electronic prescribing systems, and required applications
  • Document interactions promptly in the appropriate EMR
  • Communicate and collaborate with primary teams to ensure continuity of care
  • Support discharge planning, including medications, discharge orders, and instructions
  • Participate in QAPI activities, medical record reviews, and medical peer reviews
  • Maintain current knowledge of hospice and palliative medicine research and trends
  • Participate in departmental and medical staff activities
  • Communicate with team members, patients, families, caregivers, providers, and staff within HIPAA guidelines
  • Provide patient, family, and staff education
  • Participate in interdisciplinary-team development and implementation of patient-directed plans of care
  • Provide coverage and availability according to organizational needs
  • Meet expected productivity standards and implement telemedicine billing procedures when applicable

Requirements:

  • Minimum of 3 years of recent clinical experience, including substantial hospice, palliative care, and/or acute inpatient care experience
  • Master's or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
  • National certification, licensure, and prescriptive authority for advanced nursing practice in the home state and additional states where care is provided
  • Licensure and prescriptive authority must be obtained and maintained for all required jurisdictions
  • Applications for required licenses must be submitted within 60 days of hire and/or notification
  • Current Basic Life Support certificate from an approved organization by date of hire
  • Compact RN license in the home state by date of hire
  • Ability to work varying nights, evenings, weekends, and/or holiday shifts
  • Ability to work independently and as part of a team
  • Excellent interpersonal, collaborative, organizational, delegation, and follow-up skills
  • Ability to manage multiple tasks and stressful situations
  • Ability to work with death and dying, terminally ill patients, and highly emotional patients and families
  • Ability to work in a fast-paced, changing, high-pressure environment and meet deadlines
  • Ability to use patient health information systems, electronic medical records, electronic prescribing systems, computer telephony, and required applications
  • Proficiency with Microsoft Word, Excel, and PowerPoint
  • Expert advanced clinical judgment and problem-solving ability
  • Ability to maintain a HIPAA-compliant, temperature-controlled, noise-free work environment
  • Reliable HIPAA-compliant high-speed 100 Mb internet with hardwired backup
  • Ability to protect patient privacy and use headsets and company-approved devices, websites, and applications
  • Consent to post-offer drug and/or alcohol testing and ongoing compliance with the Drug-Free Workplace Policy
  • Florida-affiliate employees must satisfy Florida Care Provider Background Screening Clearinghouse eligibility requirements where applicable
