Job Description

Patient Coordinator Assistant supporting UK specialist weight-management and prescribing services. Coordinating referrals, assessments, patient communications, discharges, and healthcare administration.

Responsibilities:

  • Process incoming referrals across multiple services and check eligibility criteria
  • Review referral information and escalate ineligible or complex cases to the MDT
  • Monitor and manage the NHS inbox, respond to GP queries, and request additional information
  • Follow up on missed assessment appointments in line with protocol
  • Communicate with service users via phone, email, and SMS throughout their pathway
  • Follow up on outstanding forms, documentation, and incomplete tasks
  • Manage queries about appointments, group allocation, attendance, and DNAs
  • Process discharges in line with service protocols, including DNAs and non-engagement
  • Generate and send discharge confirmation letters to service users
  • Prepare and send discharge correspondence to GP practices via NHS systems
  • Offboard service users from relevant systems and accurately close patient records
  • Maintain accurate records across referral trackers and internal systems
  • Document patient activity in line with governance and audit requirements
  • Support ongoing service administration and workflow management across teams
  • Work with Patient Coordinators and clinical teams within established protocols, escalation pathways, confidentiality, and data protection requirements

Requirements:

  • Strong administrative and organisational skills, able to manage competing priorities in a busy environment
  • Excellent written and verbal communication skills, comfortable handling sensitive conversations with patients and professionals
  • High attention to detail and accuracy across data, records, and correspondence
  • Ability to handle confidential information appropriately in line with GDPR requirements
  • Confidence using digital systems and databases
  • Ability to identify risks, blockers or mistakes and raise them promptly with the right people
  • Experience in a healthcare, NHS, or patient-facing administrative role (desirable)
  • Understanding of referral pathways and healthcare processes (desirable)
  • Experience working with service users or clinical teams (desirable)
