Job Description

Patient Services Coordinator scheduling home-health visits and coordinating patient care for Graham Healthcare Group. Managing HCHB workflows, clinical staff schedules, intake information, and weekend coverage remotely.

Responsibilities:

  • Work within the Homecare Homebase (HCHB) workflow structure as directed
  • Compile daily schedules of clinical staff
  • Assign POD clinicians to patients under the direction of the Assistant Clinical Manager
  • Assist POD personnel in coordinating patient/client services
  • Serve as liaison between field staff, patients/clients, and POD personnel
  • Communicate with CFSS when order approval is holding scheduling
  • Complete daily tasks/workflows and communicate unresolved workflow issues to the Assistant Clinical Manager before shift end
  • Process visits sent back from clinicians
  • Work the required Saturday and Sunday schedule, with weekend rotation as needed
  • Accumulate admissions/intake information and communicate with CFSS about possible staffing needs and concerns

Requirements:

  • Homecare Homebase (HCHB) experience required
  • At least one year of experience in a customer service capacity
  • Minimum of two years of general office experience
  • At least one year of data entry or word processing experience
  • Proficiency in Microsoft Office Suite
  • Ability to work every Saturday and Sunday, 8 AM–5 PM EST or CST
  • Ability to work full-time, 40 hours per week
  • Legally authorized to work in the United States
  • Successful completion of a pre-employment drug screen
  • Associate’s degree preferred
  • At least one year of home health experience preferred
  • Previous home health experience preferred
