Job Description

CVS Health nurse practitioner delivering evidence-based virtual primary care for adults. Assessing, treating, counseling, and coordinating care remotely across Western U.S. states.

Responsibilities:

  • Provide evidence-based primary care for patients 18 years of age and older
  • Perform accurate, complete, concise, relevant, and appropriate history and physical assessments
  • Assess, diagnose, treat, and manage health problems
  • Provide health counseling, patient and family education, and disposition planning
  • Evaluate and treat patients’ medical needs with safe, high-quality care
  • Document patient care in the electronic health record according to policies and procedures
  • Collaborate with the dedicated care team and internal and external colleagues on patient care and coordination
  • Provide compassionate, appropriate, and effective patient-centered care
  • Refer patients to appropriate specialists and care components
  • Apply quality measurement knowledge in chronic disease management
  • Document patients wishing to join the care team panel using approved EMR workflows
  • Participate in or lead practice committees such as Epic Optimization and Care Pathways
  • Support patient care through after-hours and/or overnight call

Requirements:

  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
  • Current National Board Certification, Family Practice
  • 5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care
  • Effective verbal, written, and electronic communication skills
  • Outstanding organizational skills and ability to multi-task
  • Initiative, problem solving ability, adaptability and flexibility
  • Ability to work remotely in a care team culture
  • Proficient with information management and technology
  • Obtain and maintain multi-state licensures in good standing
  • Take after-hours and/or overnight call to support patient care
  • Broadband connectivity for remote care delivery
  • Quiet setting with a neutral background for visits
  • Ability to uphold patient privacy per CVSH guidelines
  • Completion of master’s degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required
