Job Description
In-home Nurse Practitioner or Physician Assistant providing personalized health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, conducting physical assessments and preventive screenings.
Responsibilities:
- Provide In-Home Health Evaluations in members’ homes
- Conduct approximately 45-minute visits with one-on-one health discussions
- Review medications and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening
- Use an iPad and clinical workflow for each evaluation
- Build trusted relationships and help improve health outcomes
- Visit people nationwide and connect them to appropriate care
- Provide Signify Health services, including Diagnostic & Preventive Services products
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Work primarily in private residences of health plan members
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Ability to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Ability to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
