Job Description
Registered Nurse Navigator providing remote phone triage, clinical advice, and care coordination for Geisinger patients. Assessing acuity, managing urgent needs, and supporting continuity of care.
Responsibilities:
- Provide nursing triage services and health advice by phone and in person when applicable
- Assess patient acuity and determine the need for medical support
- Provide diagnostic testing results and related education
- Manage accurate, appropriate, and timely telephone and electronic messaging
- Provide protocol-driven clinical advice
- Assess patients by phone and document chief complaints, symptoms, history, pharmacy, and contact information using established protocols
- Initiate and coordinate emergency treatment when necessary
- Prioritize urgent calls and report pertinent information to triage providers
- Educate patients and families
- Schedule or facilitate emergency appointments
- Coordinate patient care and requests across departments, specialties, and sites
- Assist with orientation and training of new personnel
- Document ongoing patient and customer communication and respond promptly to needs
- Reinforce provider instructions and treatment-plan compliance
- Explain medical conditions
- Manage patient concerns using clinical protocols and service recovery techniques
- Complete and route patient documentation in a timely manner
- Suggest process improvements and solutions
- Coordinate care and services with external organizations, including insurers, home health, and medical facilities
Requirements:
- Graduate from Specialty Training Program-Nursing (Required)
- Licensed Registered Nurse (Pennsylvania)
- Basic Life Support Certification
- 5 years of nursing experience strongly preferred
- ICU/ED/Critical Care experience strongly preferred
- BSN preferred
- Specialty certification preferred
- Proficient computer skills
- Proficient with Microsoft Word, Excel, PowerPoint, SharePoint, Windows, and Teams
- Clinical skills
- Healthcare industry knowledge
- Medical devices and tools knowledge
- Patient safety knowledge
- Ability to work rotating day and night shifts, including 7a-7:30p or 7p-7:30a
- Every other weekend and holiday rotation
- Travel of less than 25% may be required for orientation/training, meetings, or other business needs
