Job Description
Senior Program Consultant working with Trinity Health to support the National Diabetes Prevention Program. Leading initiatives to enhance prediabetes care quality measures with health partners.
Responsibilities:
- Works in collaboration with the Director, Health Promotion and Disease Prevention to support clinical delivery partners
- Leads the development of a quality dashboard and prepares reports reflecting NDPP delivery and outcomes
- Develops and manages provider engagement, develops and monitors measurable outcomes
- Improves clinical practice team’s awareness of services to support diabetes prevention in the patient population
- Develops and delivers training and technical assistance for national strategic population health quality improvement project
- Monitors quality dashboard metrics to assess progress on NDPP outcomes
- Conducts structured landscape assessments of current workflows, referral processes, and provider knowledge
- Identifies, onboards, and supports provider champions to advance program goals
- Plans, develops, and facilitates physician champion training workshops
- Engages in thought leadership and contributes to scholarly publications
Requirements:
- Must possess a seasoned knowledge of evidence and best practices for prediabetes care, NDPP program, health equity, and clinical-community referral pathways
- Minimum of seven (7) years of significant experience in program planning, implementation, management, and evaluation
- Master’s degree preferred
- At least 10+ years of experience leading clinical quality improvement initiatives
- At least 5 years’ experience working with data sets, developing data collection tools, analyzing and visualizing data
- Subject matter expertise in health disparities, social determinants of health, uncompensated and undercompensated care
- Program management skills and experience in managing large multi-site initiatives
- Experience and skills in creatively fostering a culture of collaboration, accountability, and transparency
- Strong problem-solving skills that demonstrate deadlines and achieve quality results
- Advanced organizational, analytical, presentation, interpersonal, written and verbal communication skills
