Job Description
CVS Health case manager coordinating healthcare and social support for dual-eligible Medicare and Medicaid members. Field-based travel across Will and Kankakee Counties with home-based work.
Responsibilities:
- Collaborate and inform the case management process using critical thinking and judgment
- Facilitate appropriate healthcare outcomes for members
- Assist with appointment scheduling
- Identify and assist members with accessing benefits and education
- Use care management tools and resources
- Serve dual-eligible Medicare and Medicaid members with complex health and social challenges
- Collaborate with members, providers, and community organizations
- Address members’ healthcare and social determinant needs
- Work from home while traveling up to 50-75% of the time across Will, Kankakee, and surrounding areas
Requirements:
- Must reside in the state of Illinois
- Must live in or around Will/Kankakee Counties in Illinois
- Reliable transportation and willingness and ability to travel up to 50-75% of the time from the home location in and around Will, Kankakee and surrounding areas, IL
- Minimum 3-5 years clinical practical experience
- Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
- Excellent analytical and problem-solving skills
- Effective communication, organizational, and interpersonal skills
- Ability to work independently
- Effective computer skills, including navigating multiple systems and keyboarding
- Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications
- Associate's degree required
- Active and unencumbered Registered Nurse license in the state of Illinois
- Direct/hardwired internet connection to a modem/router within 7 feet of the computer
- Minimum internet speed of 25 Mbps download and 3 Mbps upload
- Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines
- Must work within the state and city where currently residing
- Microsoft Office 365, Teams, Outlook, Word, Excel, and PowerPoint, or similar Google Workspace experience
- Ability to access secure systems such as VPN and EHR portals
- Ability to manage strong passwords and identify suspicious emails or links
- Ability to troubleshoot common technical issues
- Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience
- Preferred: Certified Case Manager
- Preferred: Bilingual
