Job Description
Medical Assistant supporting Chronic Care Management team at Nsight Health. Guiding patients through health assessments and coordinating care with nursing staff in a remote setting.
Responsibilities:
- Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls.
- Complete delegated clerical and administrative activities on behalf of nursing staff.
- Document patient interactions, assessments, and intake in the EHR.
- Maintain compliance with company policies and applicable regulations.
- Coordinate communication between patients, caregivers, nursing staff, and providers.
- Perform other duties, as assigned.
Requirements:
- Active MA license required.
- Strong communication and organizational skills.
- Bilingual Proficiency: Fluent in English and Spanish.
- Passionate about patient care, possessing the ability to relate with empathy and compassion.
- A passion for patient teaching is a must.
- Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload.
- Hardwired internet connection required.
- Speed test submission required during the offer process.

- 5% Bilingual Pay Allowance
- Competitive base pay: $18-$20/hr
- Shift Differentials: Evening Differential: +$1.50/hour for hours worked after 7:00 PM ET
- Late-Night Differential: +$2.00/hour for hours worked after 10:00 PM ET
- Weekend Differential: +$1.50/hour for all hours worked Saturday and Sunday
