Job Description
Clinical operations lead onboarding partner practices to CenterWell’s EHR, data-sharing, and population health tools. Driving value-based care adoption and optimized senior healthcare delivery.
Responsibilities:
- Design and implement initiatives that optimize clinical operations and enhance care delivery
- Report to the National President, MSO Operations
- Onboard partner practices to CenterWell tools and configure and optimize EHR systems
- Implement data-sharing strategies aligned with population health solutions and strategic ACO initiatives
- Manage practice-facing projects from start to finish
- Provide coaching and training to practice staff and drive adoption of value-based care models
- Manage multiple implementation projects, timelines, milestones, risks, and cross-functional deliverables
- Communicate project progress to practice leadership, IT staff, clinical staff, internal stakeholders, and external vendors
- Create, maintain, and update training materials
- Assess independent medical practices and recommend approaches for adopting outcomes-centric, value-based care models
- Support change management and influence adoption of CenterWell best practices
- Document implementation activities, outcomes, lessons learned, and improvement opportunities
- Ensure accurate EHR data capture for quality measures and reporting
- Design integrations and interfaces supporting data sharing and population health analytics
- Assess practice EHR usage and identify optimization opportunities
- Perform technical discovery of systems, data locations, and data-flow/integration scenarios
- Optimize workflows and help practices leverage athena, eClinicalWorks, Epic, and other EHR systems
- Support ACO quality compliance and reporting requirements
- Lead resolution of complex technical and operational challenges
- Guide practices through CenterWell resource configuration, enablement, and EHR optimization
- Develop templates, customized reports, and educational materials for initiatives such as Medicare Wellness Visits, Transitions of Care, and Advance Care Planning
- Develop and support EHR-specific user groups to identify best practices and training opportunities
- Serve as an in-house expert on 1–2 top-tier EHRs
Requirements:
- Bachelor's degree required; significant relevant work experience may be accepted in lieu of educational requirements
- 7+ years experience working as a client-facing project lead in the healthcare industry
- Proven track record of creating, maintaining, and enhancing customer relationships and optimizing EHR systems
- 3+ years implementation or optimization experience with top-tier ambulatory EHRs and practice management software
- Preferably expertise in Epic Ambulatory, NextGen, Athena, or eClinicalWorks products
- Ability to work remotely within the US
- Ability to travel periodically to partner practices and/or CenterWell meetings
- Self-motivated and open to learning new EHR platforms/workflows
- Organizational, project management, critical thinking, problem-solving, and attention-to-detail abilities
- Effective written and verbal communication
- Ability to prioritize and manage multiple projects in a fast-paced, dynamic environment
- Ability to lead and mentor team members
- Experience developing and implementing innovative solutions in complex healthcare environments
- Microsoft 360 suite proficiency
- Development experience with PowerApps and PowerBI
- Home internet with minimum 25 Mbps download and 10 Mbps upload speeds
- Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
