Job Description
Clinical Pharmacist optimizing medication regimens and safety for AnewHealth’s complex-care patients. Conducting reviews, resolving medication problems, and collaborating with providers and care teams.
Responsibilities:
- Conduct comprehensive and targeted Medication Safety Reviews for PACE participants using proprietary clinical platforms
- Develop individualized, measurable, goal-oriented therapeutic care plans
- Complete medication reviews and reconciliation for new enrollments, annual and significant-change assessments, care transitions, and high-risk clinical events
- Identify, evaluate, document, and facilitate resolution of medication-related problems
- Collaborate with providers, interdisciplinary team members, pharmacists, and other healthcare professionals
- Communicate medication optimization, deprescribing, and goals-of-care recommendations
- Engage providers through medication safety reviews, clinical consultations, polypharmacy assessments, fall-risk evaluations, and care-transition reviews
- Monitor and document outcomes and progress toward therapeutic goals
- Monitor participants receiving high-risk medications or managing complex chronic-disease regimens
- Participate in client meetings, interdisciplinary discussions, P&T Committee meetings, drug-utilization reviews, quality-improvement initiatives, and educational programs
- Maintain compliance with pharmacy regulations, accreditation standards, HIPAA, Fraud, Waste, and Abuse requirements, and patient-safety reporting
- Travel may be required for organizational events, team meetings, trade shows, conferences, and client engagements
Requirements:
- Graduate of a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE), or a qualifying pharmacy program outside the U.S.
- Current, active license/registration in good standing with the state board of pharmacy where practice will occur
- Active Board of Pharmacy Specialties certification in geriatrics (BCGP) preferred
- Direct contact experience with prescribers and patients preferred
- Experience or strong interest in clinical pharmacogenomics preferred
- Above-average knowledge of geriatric pharmacotherapy
- Ability to apply geriatric pharmacotherapy knowledge to patients with concurrent illnesses and multiple medications
- Ability to resolve complex medication-related issues
- Strong analytical skills, clinical judgment, and decision-making ability
- Strong technical and computer skills, including pharmacy software
- Demonstrated problem-solving, conflict-resolution, and project-management skills
- Effective written, verbal, and interpersonal communication skills
- Detail-oriented and organized; able to multitask, work under pressure, and meet deadlines
- Knowledge of state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse
- Availability to work an average of 40 hours per week and outside normal business hours on an ad-hoc basis
