Job Description
Consultative Pharmacist providing optimal medication therapy and patient care consultation. Engaging with patients and providers to ensure safe medication usage and health improvement.
Responsibilities:
- The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy
- Advises safe, accurate and timely usage of medications
- Designs, implements, evaluates and makes adjustments to medication care plans
- Monitors medication therapy and makes adjustments as needed
- Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions
Requirements:
- Bachelors Degree
- Pharmacist license in good standing in OH, AZ, Fl or TX
- Interact with CenterWell Pharmacy patients via the telephone
- Provide counseling and advice
- Research patient concerns
- Facilitate positive service and pharmaceutical outcomes
