Job Description

Health Data Specialist monitoring OhioRISE outcomes and reporting for I Am Boundless, an Ohio nonprofit supporting people with I/DD. Ensuring care coordination activities and documentation are complete.

Responsibilities:

  • Gather, monitor, and report outcomes data and activity monitoring related to required OhioRISE CME activities
  • Monitor a large population of individuals and families/caregivers supported by care coordination and care management activities
  • Ensure all required activities and reporting are complete for individuals in the assigned population pool
  • Work with assigned care coordinators to address gaps
  • Obtain or exchange necessary records or documentation with staff from provider or community organizations
  • Communicate primarily with care coordination teams via telephone, video conferencing, and electronic communication

Requirements:

  • High School or GED required
  • One to three years of experience working with health data, data analytics, health outcomes reporting, or data reporting tools
  • Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles
  • Proof of auto insurance
  • No more than 5 points on license
  • Successful completion of background check, drug screen, and health screen, as applicable for the role
