Job Description
In-home Nurse Practitioner or Physician Assistant delivering health evaluations for CVS Health’s Signify Health. Reviewing medical histories, performing physical evaluations, and connecting members to appropriate care.
Responsibilities:
- Provide in-home health evaluations to health plan members
- Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation
- Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Build trusted relationships and help improve health outcomes
- Visit people nationwide and connect them to appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams and bone density screening
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Ability to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Ability to safely perform duties in homes with household pets, secondhand smoke and other potential airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
