Job Description

Registered Nurse supporting Color’s nationwide Virtual Cancer Clinic through cancer screening triage, symptom follow-up, and care coordination. Providing empathetic telehealth education and patient support through survivorship.

Responsibilities:

  • Serve as a clinical resource for the cancer screening and prevention population through triage, care coordination, and education via phone and asynchronous messaging
  • Conduct clinical and needs assessments
  • Perform telephone visits for patients reporting symptoms
  • Coordinate follow-up care and escalate patients to the next level of care when needed
  • Follow up on abnormal cancer screening results, educate patients on next steps, and coordinate follow-up appointments until workup completion
  • Review annual patient engagement surveys for changed screening needs and new clinical needs
  • Provide compassionate, empathetic, trust-based patient care
  • Collaborate with Care Advocates on non-clinical support and care needs
  • Develop clinical protocols and best practices for care coordination from screening through survivorship
  • Engage patients regarding symptom management, care escalations, provider referrals, and in-person care planning
  • Participate in care team sessions to monitor member progress and develop targeted support plans
  • Maintain thorough and detailed encounter documentation
  • Partner with physicians, advanced practice providers, genetic counselors, and care advocates

Requirements:

  • Bachelor of Science in Nursing (BSN) required
  • Unrestricted active Registered Nurse (RN) license, with eligibility to obtain additional state licenses as needed
  • Related certification required or to be obtained within 12 months of hire, such as AMB-C, CCM, CMGT-BC™, OCN, AOCN, or AOCNS
  • Minimum of 7 years of nursing experience
  • At least 5 years in primary care with result follow-up experience
  • Outpatient experience required
  • Inpatient and/or oncology experience are pluses
  • Strong organizational and multitasking skills
  • Strong verbal and written communication skills, with empathy and active listening
  • Comfort with video chatting, electronic health records, and other telehealth tools
  • Professional fluency in English and Spanish, including medical Spanish, strongly preferred
  • Regular availability during Pacific Time business hours
  • Unrestricted right to work in the United States
