Job Description
Remote LPN supporting Amberwell Health, a healthcare system providing comprehensive and primary care. Coordinating EMR tasks, patient education, referrals, authorizations, and chronic-care follow-up.
Responsibilities:
- Assist onsite MA, LPN, and RN in collaboration with the provider and healthcare team through the EMR
- Work the patient task list through the EMR
- Communicate clinical results and education to patients by phone or portal and document in the EMR
- Perform medication prior authorizations
- Document patient/family interactions, medications, and test results in medical records
- Assist the care team with nursing tasks through the EMR
- Communicate closely with nursing and provider teams
- Ensure patient information is accurately located in charts for quality reporting and data extraction
- Coordinate and manage provider-seen patient care, including tracking testing and referrals
- Monitor insurance requirements and guidelines
- Act as patient advocate
- Monitor patients to promote wellness, health maintenance, preventive care, and chronic care
- Provide patient education materials and communicate provider advice and instructions
- Schedule, coordinate, and communicate specialist referrals and provider-ordered services
Requirements:
- Ability to multi-task
- Good organization skills
- Effective communication skills
- Completion of accredited Vocational/Technical Nursing Program
- Unencumbered active Kansas nursing license
- BLS certification
- Preferred: 1–2-year experience working in a clinic setting
