Job Description
Part-time nurse practitioner delivering coordinated geriatric care for long-term care residents. Assessing conditions, prescribing treatments, and improving outcomes across nursing homes and assisted living communities.
Responsibilities:
- Assess patients’ medical and mental health needs
- Provide plans of care that proactively manage patient conditions and address barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services
- Prescribe medications
- Order lab work, diagnostic procedures, and consultations
- Monitor patients’ compliance and response to treatment
- Modify treatment plans as needed
- Collaborate with providers, case managers, facility partners, and health plan leadership
- Implement Treat in Place Protocols
- May provide training and advice to facility staff
Requirements:
- APRN or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehabilitation with geriatric populations and/or Fee for Service experience
- Experience with Electronic Health Records
- Working knowledge of Microsoft applications, including Word, Outlook, and Excel
- Bachelor’s or master’s degree preferred
- Ability to provide independent judgment in treating acute and chronic conditions
