Job Description

Part-time Nurse Practitioner providing proactive geriatric care for TruHealth’s long-term care Medicare Advantage members. Managing acute and chronic conditions across nursing homes and assisted living communities.

Responsibilities:

  • Assess patients’ medical and mental health needs
  • Provide proactive plans of care that manage conditions and address barriers to care
  • Ensure compliance with local, state, and federal agencies related to clinical services
  • Prescribe medications
  • Order lab work, diagnostic procedures, and consultations
  • Monitor patients’ compliance and response to treatment
  • Modify treatment plans as needed
  • Work with RN case managers as part of an integrated care team
  • Collaborate with providers, case managers, facility partners, and health plan leadership
  • Implement TruHealth’s national model of care in nursing homes or assisted living communities
  • Provide training and advice to facility staff as required

Requirements:

  • FNP, AGNP, AHACNP or PA license required
  • Degree from an accredited APRN or PA program
  • 3 years’ experience in clinical nursing or rehab in geriatric populations
  • Electronic Health Records experience
  • Working knowledge of Microsoft applications, including Word, Outlook and Excel
  • Master’s degree or better
  • Primary care in geriatric medicine experience preferred
  • Long-term care experience preferred
  • APRN Advanced Practice RN certification/license required
  • Ability to comply with federal and state regulations
  • Ability to exercise independent judgment and consult with a supervising physician as appropriate
