Job Description
Part-time Nurse Practitioner providing proactive geriatric care for TruHealth’s long-term care Medicare Advantage members. Managing acute and chronic conditions across nursing homes and assisted living communities.
Responsibilities:
- Assess patients’ medical and mental health needs
- Provide proactive plans of care that manage conditions and address barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services
- Prescribe medications
- Order lab work, diagnostic procedures, and consultations
- Monitor patients’ compliance and response to treatment
- Modify treatment plans as needed
- Work with RN case managers as part of an integrated care team
- Collaborate with providers, case managers, facility partners, and health plan leadership
- Implement TruHealth’s national model of care in nursing homes or assisted living communities
- Provide training and advice to facility staff as required
Requirements:
- FNP, AGNP, AHACNP or PA license required
- Degree from an accredited APRN or PA program
- 3 years’ experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
- Master’s degree or better
- Primary care in geriatric medicine experience preferred
- Long-term care experience preferred
- APRN Advanced Practice RN certification/license required
- Ability to comply with federal and state regulations
- Ability to exercise independent judgment and consult with a supervising physician as appropriate
