Job Description
Practice Success Manager growing Guidehealth’s value-based healthcare practice partnerships. Managing onboarding, performance reviews, retention, and clinical-service coordination across Georgia.
Responsibilities:
- Identify and evaluate individual practices within assigned networks for fit with Guidehealth’s value-based care services
- Conduct outreach and initial meetings with practice leadership to introduce Guidehealth’s services and value proposition
- Build the case for engagement, address objections, and secure agreement to enter implementation
- Coordinate engaged practices’ kickoff and onboarding with the Practice Operations Manager
- Serve as the day-to-day point of contact for an assigned portfolio of practices
- Maintain consistent communication and touchpoint cadence with practice administrators and frontline clinical staff
- Support long-term practice retention and satisfaction
- Represent practice feedback, needs, and risks internally, routing or escalating them as needed
- Document practice meetings, key correspondence, and interactions in HubSpot CRM
- Schedule and coordinate performance reviews, escalation discussions, and ad-hoc check-ins
- Determine appropriate meeting cadence and format based on relationship stage, performance trajectory, and practice preference
- Provide monthly performance reviews and facilitate conversations with practice leadership
- Translate performance data and benchmarks into actionable next steps
- Support practices in developing and tracking improvement plans tied to performance targets
- Identify underperforming practices and escalate supporting context to the Director
- Liaise with Guidehealth’s clinical pillars: CDI, Care Management, and Quality
- Relay practice feedback, updates, and operational conditions to internal clinical leads
- Represent Guidehealth’s services and expectations during implementation kickoff and onboarding
- Resolve internal barriers or delays affecting practice service delivery
- Collaborate with the network’s Client Relationship Manager when practices are unresponsive or additional support is needed
- Support the Director of Practice Success with network- or client-level presentations and business reviews
- Contribute portfolio-level performance narratives and practice-level context
- Keep the Director informed of network trends, risks, and practice sentiment
Requirements:
- Ability to travel locally up to 50%
- Must reside locally in Georgia
- Minimum 3 years of experience in a practice-facing, account management, or business development role within healthcare, value-based care, or a related field
- Experience working within an ACO, MSSP, or value-based care model, including delegated services, managed care, or population health environments
- Ability to build trust-based relationships and secure buy-in from physicians and practice leadership
- Ability to facilitate productive, sometimes difficult, performance conversations with practice leadership
- Strong understanding of individual primary care physician practice workflows, day-to-day operations, and front-office/clinical environments
- Ability to manage multiple practice relationships simultaneously with high attention to follow-through
- Strong written and verbal communication skills, including translating performance data into a clear narrative for practice audiences
- Strong time and task management skills
- Judgment to calibrate engagement intensity and format across practices with differing needs
- Ability to work independently with strong ownership and accountability for practice-level outcomes
- Based in or willing to relocate to the Atlanta, GA area
- Ability to travel up to 50% of the time within Georgia for in-person practice engagement
- Experience specifically in practice acquisition, physician outreach, or provider network development is preferred
- Clinical background, such as Medical Assistant, Care Manager, RN, or LPN, is preferred
- Experience in a manager or operations role within a primary care practice is preferred
- Health coach certification or motivational interviewing skills are preferred
- Experience with CRM platforms, preferably HubSpot
- Familiarity with quality metrics, HCC coding, or care management integration
- Familiarity with the Atlanta-area independent physician practice landscape
- Ability to work during stated working hours and maintain a camera-on presence for virtual meetings
- Internet connection of at least 100 Mbps download and 10 Mbps upload
