Job Description
Clinical pharmacist managing medication therapy for Conviva Senior Primary Care seniors. Supporting chronic-condition care, patient education, quality measures, and physician teams remotely.
Responsibilities:
- Evaluate pharmacotherapy suitability for high-risk patients with chronic conditions including diabetes, hypertension, heart failure, and COPD
- Execute clinical protocols within organizational and state regulatory requirements
- Optimize drug coverage benefits through preferred formulary products, lower-cost alternatives, and home delivery pharmacy
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care
- Participate in multidisciplinary high-risk rounds and adjust treatment plans
- Counsel patients and communicate complex medical information to patients and healthcare providers
- Provide disease-state education on medication use and self-management
- Engage in medication safety and efficacy quality-improvement initiatives
- Perform post-discharge medication reconciliation for high-risk transition-of-care patients
- Document clinical activity in the clinic EMR and other pharmacy or clinical management systems
- Support CMS Stars and HEDIS quality measures
- Maintain documentation of clinical interventions and patient care activities
- Conduct in-service education and training for healthcare staff
- Develop and share pharmacotherapy educational materials
- Stay current with pharmacotherapy advances and clinical guidelines
- Consult healthcare providers on complex medication management issues
- Provide support within the pharmacist's scope of practice to advance organizational goals
- Implement targeted initiatives directed by Pharmacy leadership according to best practices and regulatory standards
Requirements:
- Active and unrestricted pharmacist license in the state of Florida
- Willingness to become licensed in additional states for market support
- Pharm.D. or other advanced pharmacy degree
- Minimum five (5) years of experience as a practicing pharmacist
- Experience working in a team environment and supporting common goals
- Proficiency with MS Office programs including Word, Excel, PowerPoint, Teams, and Outlook
- Ability to troubleshoot and resolve basic technical needs within a remote environment
- Multilingual, preferably English/Spanish; must pass both language assessments
- Clinical residency in ambulatory care or similar discipline preferred
- Board certification in geriatric pharmacy preferred
- Experience in an ambulatory care/primary practice setting preferred
- Familiarity with electronic medical record (EMR) systems preferred
- Experience managing drug therapy for patients with chronic conditions preferred
- Must reside within a 50-mile radius of Corpus Christi or El Paso
- Ability to work Monday-Friday, 8:00 a.m.-5:00 p.m. Eastern
- Home internet with at least 25 Mbps download and 10 Mbps upload
- Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
- Willingness to travel approximately 5% for market meetings and as needed
