Job Description

Clinical Pharmacist improving HEDIS medication adherence for Guidehealth, a data-powered healthcare company. Analyzing quality data, engaging patients, and supporting value-based care initiatives.

Responsibilities:

  • Lead efforts to improve HEDIS medication adherence and pharmacy quality measures
  • Analyze medication adherence data, identify care gaps, and implement targeted interventions
  • Collaborate with healthcare providers, care managers, and stakeholders on adherence strategies
  • Conduct medication therapy management sessions and provide patient education
  • Identify and resolve adherence barriers including cost, side effects, and complex dosing schedules
  • Use motivational interviewing to encourage medication adherence
  • Monitor and track HEDIS performance metrics
  • Generate reports evaluating intervention and program effectiveness
  • Use EHR and pharmacy data to identify patients at risk of non-adherence and develop action plans
  • Provide leadership with updates and recommendations toward HEDIS goals
  • Serve as the organization’s medication adherence subject matter expert
  • Train and support pharmacists, care managers, and clinical staff
  • Participate in quality improvement, population health management, and value-based care initiatives
  • Follow security policies and procedures to protect PHI, PII, and company intellectual property

Requirements:

  • Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
  • Active pharmacist license
  • Willingness to obtain licenses in TX, TN, AL, and PA, company paid
  • Minimum of 2 years of experience in HEDIS-related medication adherence work, quality improvement, or population health
  • Strong knowledge of HEDIS measures, particularly medication adherence measures
  • Proficiency using EHR systems and data analysis tools
  • Excellent communication, problem-solving, and organizational skills
  • Ability to work independently and in a team-oriented environment
  • Experience with motivational interviewing or other patient engagement techniques is a plus
  • Experience in a managed care setting, PBM, or healthcare quality improvement role is preferred
  • Certification in MTM, CGP, or other relevant credentials is preferred
  • Understanding of value-based care models and their impact on medication adherence is preferred
  • Employees must provide their own internet connection with at least 100 Mbps download and 10 Mbps upload
