Job Description
Remote clinical pharmacist verifying prescriptions and counseling patients for Advocate Health’s Illinois and Wisconsin pharmacy operations. Supporting medication safety, consultation, education, and hospital communication.
Responsibilities:
- Work remotely to review, verify, and counsel on Meds to Go prescriptions in Illinois
- Assist with hospital communication and education
- Provide verification and consultation for Medifriend locations and Wisconsin RD pharmacies
- Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
- Counsel patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and administration
- Design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens
- Review and accurately interpret medication orders and prescriptions
- Participate in medical emergencies when applicable
- Support medication safety and quality improvement activities, including event reporting
- Perform quality assurance and performance improvement of the medication-use system
- Collaborate with the healthcare team on formulary adherence and the medication-use process
- Assist with non-formulary, compassionate-use, and investigational medications
- Prepare, dispense, and oversee proper medication storage and handling
- Serve as preceptor for pharmacy students, residents, and healthcare learners
- Mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students
- Direct and check pharmacy support team work for safe, efficient, compliant workflows
- Provide age-appropriate patient care based on growth, development, and patient-status data
Requirements:
- Licensed in the state(s) of practice or eligible for reciprocity required
- Immunization and non-vaccine injectable certification required based on area of practice
- Credentialing and privileging required based on area of practice
- Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP
- No experience required
- Maintains skills in area of practice
- Strong computer skills
- Excellent communication and organization skills
- Excellent presentation, facilitation, and training development skills
- Decision making, team building, customer service, and conflict resolution skills
- Life support training courses may be required dependent on department discretion
- Must be licensed in both Illinois and Wisconsin
- Ability to work 1st and 2nd shifts and weekends
- Ability to meet physical requirements including walking, standing, pushing up to 50 pounds, and lifting 35 pounds
- Ability to use pharmacy, computer, and automated dispensing equipment
- Functional speech, vision, and hearing
