Job Description
Remote licensed social worker managing patient transitions and coordinating with health plans, medical groups, and community partners. Supporting Providence’s patient-focused healthcare services across California.
Responsibilities:
- Communicate with patients, health plans, and medical groups
- Work directly with patients and key personnel at each ministry, Care Management, insurance companies, and community partners as needed
- Provide excellent customer service
- Exercise initiative and judgment in a busy environment
- Follow up and follow through with patients and stakeholders
- Maintain thorough knowledge of services throughout the Providence continuum
- Act as a resource to the community
Requirements:
- Master's Degree Social Work
- Upon hire: California Clinical Social Worker License or California Marriage and Family Therapist License
- Five years previous experience working in a healthcare role is required
- Practical experience in customer service
- Background in managed care health insurance or care management preferred
