Job Description
Clinical Social Worker partnering with nurses to coordinate member case management for CVS Health. Providing telephonic advocacy and community-resource support from North Carolina.
Responsibilities:
- Partner with a registered nurse case manager in a collaborative process of assessment, planning, and advocacy
- Meet members’ benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Provide telephonic and electronic member contact across the entire lifecycle population
- Empower members to make positive lifestyle and medical choices
- Collaborate with clinical staff to assist members with medical, financial, and behavioral health community resources
- Work Monday-Friday 8am-4:30pm EST
- Provide no weekend, holiday, or travel coverage
Requirements:
- Must live in North Carolina
- Master's Degree in Social Work
- Active, unrestricted licensure as one of the following: LSW (Licensed Social Worker), LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), LISW (Licensed Independent Social Worker)
- 2+ years of experience in medical social work or case management
- Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training
- Must be able to work EST hours
- Monday-Friday 8am-4:30pm EST availability
