Job Description
Supervising multidisciplinary Innovations Waiver care-management teams at Trillium Health Resources, a North Carolina managed-care organization. Overseeing person-centered support for members with I/DD and traumatic brain injury.
Responsibilities:
- Provide direct supervision and oversight for an assigned team of localized, multidisciplinary professionals supporting members with intellectual/developmental disabilities or traumatic brain injury
- Ensure staff use Trillium Connections workflows to complete comprehensive care-management assessments and care plans as members’ care needs change
- Establish a team-based, person-centered approach to coordinating members’ physical health, behavioral health, and social-determinant-of-health needs
- Ensure care coordination is performed effectively, efficiently, and in accordance with NCQA procedures
- Monitor teams to help ensure integrated care through multidisciplinary collaboration with members, caretakers/legal guardians, PCPs, behavioral-health and IDD providers, specialists, nutritionists, pharmacists, and others
- Apply evidence-based and industry-standard practices to develop processes, procedures, and workflows for effective care coordination and management
Requirements:
- Bachelor’s degree or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI; OR Master’s degree in a human services field or licensure as an RN, and three (3) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI
- Valid driver’s license
- Must reside in North Carolina and within 75 miles of Trillium’s Midstate Region, including Anson, Guilford, Montgomery, Randolph, and Richmond counties
- Must be able to travel within catchment as required
- Work experience focused on working with Innovations Waiver (preferred)
- Minimum of two (2) years of experience supervising a team (preferred)
- All candidates must pass a drug test as a condition of employment
